Going through it very slowly. Plenty screenshots, so not in line.
As a starting point, I have an existing square brush but I do not have any presets. Might not be your situation in which case you can skip the first steps.
1. Using square brush set to size.
2. Click on Save Tool Preset...
3. That brings up the default preset editor dialogue.

4. Give it a name
5. Check the options you want
6. Save it.

7. Set brush to new size, click on New Tool Preset
8. You should have the default preset editor dialogue back

9. Repeat of earlier - give it a name
10. Set the options
11. Save it

12. Check that the presets are there http://i.imgur.com/bFpOi9m.jpg
If stuck, remember that the presets are text files, held in your Gimp profile and can be copied and edited in a suitable text editor. You can also delete any you do not want when you screw up
