For the streetlight at the background on the shoulder/neck I would use the clone tool to remove it definitively, you're right it's distracting.
for other reflection, try with the "healing tool"
In all case that's really noisy....

EDIT: OK, rich2005 did an edit, its last edit is an excellent example of what you can achieve on those street lights and reflected lights
