How to display Toolbox

But the icons are too small to read.  How to increase their size?

You don't read post nor screenshot fully? Look at the third arrow pointing down on my screenshot... click on that button, chose an option, if "Custom" the slider below will highlight active, move that slider...

I already select "Custom icon size"

The slide seems not working.  I can't select Medium/Large/Huge
